Field Medical Card also known as Army Form W3118 was gathering information about the treatment the soldier has received. Used by Royal Army Medical Corps. They were stored in special envelopes and kept by the soldier until the treatment was over. This one was issued to a Polish soldier – Jan Krasicki.
